Requirements:

  • The applicant must be at least 23 years of age
  • The applicant must have 2 years of verifiable experience and must have experience with the type of trailer to be driven
  • The applicant must have a current physical and supply a copy of the long-form that has been registered with the DMV and completed by a physician registered with FMCSA
  • The applicant must have a valid CDL in the state of residence
  • The applicant must not have had a DUI in the previous 3 years
  • The applicant must not have had more than one DOT recordable preventable accident in the last 3 years
